Connect HomeTeams to your AI assistant
HomeTeams is where families coordinate care for the people they love — aging parents, a spouse in recovery, a child with complex needs. The HomeTeams connector brings your care team into AI assistants such as Anthropic's Claude, so your assistant can work with your team on your behalf.
What you can do
Ask what's happening this week and get the real answer from your team's shared board: who completed what, what's overdue, and what's coming up. Add an action ("pick up Dad's prescriptions Saturday") and assign it to a teammate without opening the app. Mark things done. When you need a human, ask your team — your assistant posts your question into the team chat, attributed to you, and that is the only way anything here reaches another person.
- See the week at a glance. Ask what's on your care team this week and get the shared board's real state: open actions, who's doing what, what's overdue.
- Delegate in the moment. "Add an action to refill Mom's prescriptions Saturday, assign it to my sister" — it appears on the team board with the right assignee and date.
- Ask the team. "Who can drive Tuesday?" — your assistant posts the question into the team chat, attributed to you.
- Check health information before an appointment. With the Health permission, review a medication list or ask a question of imported health records ("When was Mom's last A1C?").
Permissions
There are two permission levels, and each is shown to you before you approve:
- Care team and actions — your team list, shared actions and reminders, team activity, and the ability to create or complete actions and post a question into your team chat, attributed to you.
- Health information — granted separately on the consent page. Your assistant can answer from health information you already have access to in HomeTeams: current medication lists, imported health records (visits, labs, claims), and documents in your team's vault. Everything is scoped to your own account's permissions, profile by profile and folder by folder. If a folder isn't shared with you in the app, it isn't visible here.
HomeTeams returns your information; it holds no drug-interaction data and never makes clinical judgments. The connection never widens your access.
What you need before connecting
A HomeTeams account (iOS or Android app) that belongs to at least one care team, and the phone that receives your HomeTeams sign-in codes. Sign-in is the same phone verification the app uses.
Privacy and control
Connections are optional, off by default, and only you can create one for your account. You can disconnect an assistant at any time in that assistant's settings, which immediately revokes its access — or email support@hometeams.io and we'll revoke it from our side.
