HomeTeams is the system that connects families, communities, and healthcare for the first time.

A team in action, working together with purpose
Schedules, medical appointments, meals, and errands live in one shared place.
The cousin who checks in, the neighbor who drives, the nurse who calls back. Everyone sees their part of one shared picture, and you decide who sees what.
Medications, appointments, and the questions in between stop landing on one person.
One shared picture of schedules, tasks, and who has what covered.
KIDS · FAMILIESMedications, appointments, and the whole team on the same page.
SENIOR LIFE · CRITICAL CONDITIONSHabits, goals, and the moments worth celebrating, shared with your team.
HEALTHY AGING · EVERYDAY WELLNESSAI guidance + your care team, whenever you need it.
Everyone knows what needs to happen next.
Preserve stories, voices, and moments for generations.
Every important record, ready when you need it.
When a family team is part of the care plan, patients are less likely to end up back in the hospital within 90 days.
Caregivers who go it alone average 22.8 hours of care work every week. With a HomeTeam, that load is spread across many hands.
Memory care can run $10,000 a month. With a HomeTeam in place, families can bring that down to around $2,700 by coordinating care at home.

Answers the question in front of you at 11pm, in plain words. It knows the plan, the medications, and who on your team to pull in.
FOR THE PERSON HOLDING IT ALL TOGETHERMedical records, labs, medications, clinical notes, and the things only your family knows, together in one secure picture you control. Coach AI helps your team understand what matters and ask better questions.
KNOW MORE · ASK BETTER · STAY IN CONTROLInvite the people already helping and give each a clear role. Everyone sees their part of one shared picture.
FOR FAMILY, FRIENDS, AND CLINICIANSDaily tracking and medication adherence carried by the whole team instead of one exhausted person.
FOR FAMILIES MANAGING A LONG-TERM CONDITIONBrae programs build the physical, mental, and spiritual strength to keep showing up.
FOR THE STRENGTH TO KEEP CARINGThe cost of care lands on the person least able to plan for it. So everyday purchases at participating businesses can send something back to your family's HomeTeams Wallet, ready to use for care, or to share with someone you love.
EARN · SHARE · CAREA life is more than a medical chart. The stories, the voice, the year they almost lost the farm: recorded together, while everyone is still at the table to hear them.
REMEMBER · UNDERSTAND · CARRY FORWARD"It took the total collapse of my health, and, for that matter, my life, for me to understand this: caring is the center of life. And caring may be the healthiest activity in life."
Years of concussions, illness, and seizures stripped away my health and my belief that I could find my way back. During one seizure in the Wallowas, Poppy, my dog, stood watch over me through the night and led me back to my son. That collapse taught me that survival is rarely solo, and that care is not what happens after medicine. It is the family member who coordinates, the friend who stays, and the team that carries hope when yours runs thin. HomeTeams was built so none of them has to carry it alone.
Answers the question in front of you at 11pm, in plain words. Coach knows the plan, the medications, and who on your team to pull in.
A real Coach conversation: a scary number becomes a plain answer, then a next step.
Ask anything, anytime. Type or just talk.
Get a plain-English answer grounded in your person's actual records and plan.
Coach turns it into action: a flagged result, a question list, the right person pulled in.
Medical language isn't built for families. Results and options come back dense, jargon-heavy, and hard to act on.
Questions don't keep office hours. The scary ones show up at night, between visits, when no one picks up.
Answers without context don't help. A generic search knows nothing about your person, their meds, or their plan.
Chat with Coach AI anytime, day or night. No appointment, no hold music.
Labs, imaging, and visit notes explained in plain English, with next steps.
Talk naturally instead of typing. Hands full is the normal state of caregiving.
Coach knows your plan and suggests who on the team to pull in, and when.
Important reminders and updates arrive automatically. Nothing to chase.
Join a health program for body, mind, and spirit, whether you're giving care or receiving it.
Ask anything in any language: health questions, scheduling help, whatever's in front of you. Like talking to a trusted friend.
Ask Coach for a summary of what the team has handled. Catch up in seconds instead of scrolling.
Your conversations and data are encrypted in transit and at rest.
Built to meet HIPAA privacy and security standards.
Granular, per-person permissions on everything you share.
Coach answers from your person's actual records and plan, and knows when to hand off to a human.
One HomeTeam, every device. Free to download.
Only about 1 in 8 U.S. adults has proficient health literacy; the rest routinely leave appointments with results they can't fully act on. A translator changes what families can do.
Patients invited to read their doctors' notes through a portal showed measurably better medication adherence than those without access.
The HomeTeams Vault is a secure home for everything that matters about your health: every doctor, hospital, lab, and clinic in one place you own. One vault, not a dozen portals.
The real Health screen: synced devices, organized folders, and a nurse one tap away.
Connect a device. Steps and sleep sync automatically from Apple Health, Google, or Fitbit.
Everything lives in folders (Medications, Medical history, Resource library) with per-person permissions.
Need a human? Call a 24/7 nurse or schedule a provider visit right from this screen.
Records live everywhere but one place. Every doctor, hospital, lab, and clinic keeps its own piece of the picture, never the whole thing.
Medical language isn't built for families. Lab results and treatment options come back dense and hard to act on.
One person carries it alone. Without a shared place to organize and share, the burden of “keeping track” falls on one family member.
Collect, organize, and own your medical and health records from every doctor, hospital, lab, and clinic.
Family, caregivers, and providers see exactly what you choose: full access or view-and-comment, per person, per record.
From lab results to imaging, medication lists to visit notes: your complete health history, always at your fingertips.
Coach AI translates medical language into clear, plain-English explanations.
Understand test results and treatment options, with next steps you can act on.
Coach AI monitors new research so you don't have to, keeping insights current for you and your family.
One vault, not a dozen portals. Every record lives in a single, secure home you actually control.
An interpreter, not just an inbox. Coach AI turns medical jargon into decisions you can make with confidence.
Built for the whole family. Track and understand research and records for every member of your HomeTeams.
You choose who sees what. Grant full access or view-and-comment access, per person, per record.
Health data translated into a clear, shared picture your whole team can actually use, not raw numbers.
Connect with a nurse anytime or schedule a provider visit right from the Health screen. Support is never more than a tap away. GLP telehealth appointments and prescriptions coming soon.
Guides, routines, meal plans, therapy exercises, and videos, organized in folders, searchable, with per-person permissions.
Diagnoses, lab results, insurance cards, medication lists, and specialist notes, there when it matters most, with access you control.
Records are encrypted in transit and at rest.
Built to meet HIPAA privacy and security standards.
Folder-level permissions, per person, per record.
Imports from portals, PDFs, and photos of paper records, organized automatically.
One HomeTeam, every device. Free to download.
Studies in a systematic review found that giving patients access to their own records improved outcomes, from medication adherence to blood pressure and glycemic control.
Patients with access to their records catch medication-list errors and discrepancies their clinicians miss. Record access is a safety feature, not just a convenience.
Build a caregiving team from the people already in your life (family, friends, neighbors, community) so no one carries it alone. Everyone sees their part of one shared picture.
The Home dashboard: the team’s week getting claimed and checked off in real time.
The dashboard shows where attention is needed: one shared picture for the whole team.
Actions update in real time. Tap any card to leave a note or loop in a teammate.
Team Momentum makes sure every contribution is seen and appreciated.
One person carries it alone. Care naturally lands on whoever is closest, and quietly burns them out.
Coordination lives in a dozen threads. Group texts, phone calls, and sticky notes. Nobody sees the whole schedule.
Willing helpers can't help. The cousin who checks in and the neighbor who drives want a part. There's just no way to hand them one.
Invite family, friends, neighbors, and caregivers in minutes, each with a clear role and a customizable access level.
One caregiving calendar keeps every team member coordinated: visits, drives, meals, and appointments.
Everyone knows what happens next: who's on it, what's due, and what's done.
Team chats and direct messages keep conversations in one place, with less miscommunication, fewer dropped handoffs.
Updates the whole team can follow, so nobody has to ask “what did I miss?”
Notifications and reminders from your care team arrive on their own. Nothing to chase.
Built for caregiving, not just chatting. Schedules, actions, and conversations live together, tied to the person you love.
You decide who sees what. Access levels per member keep private things private.
The load actually spreads. When helping is easy to hand out, more people help.
Not just for seniors. Co-parents splitting schedules, families managing a child's condition, siblings coordinating from different cities: any home with more than one helper.
Everything in one place, so the whole team always knows where attention is needed. Team Momentum makes sure every contribution is seen and appreciated.
The shared hub that keeps everyone on the same page. Every action updates in real time. Tap any card to dig in, leave a note, or loop in a teammate.
A highlighted team chat plus direct messages. Search past messages; hold a message to react, reply, or copy.
For care that happens at a specific time: meds, appointments, check-ins, meals, rides. Set a schedule, attach notes or photos, and require check-ins from the teammates responsible.
For flexible tasks with no set time: errands, supplies, household jobs. A shared list so nothing important gets forgotten.
Add a medication once (details, schedule, and assigned teammates) and it appears in your team's Actions so it's always given correctly.
Chats, schedules, and records are encrypted in transit and at rest.
Built to meet HIPAA privacy and security standards.
Access levels per member: the neighbor sees rides, not records.
Schedules, actions, and chats update instantly for every member.

Fewer hospital readmissions within 90 days when family caregivers are integrated into discharge planning, and 24% fewer at 180 days, across 11 randomized trials of older adults.
The average week of unpaid care a solo caregiver carries. Distributing even part of it across a team changes what any one person has to hold.
Coming soon: your personal research powerhouse. It gathers, analyzes, and synthesizes everything that matters about your health, then hands you a plan you can use with your doctors and your team.
A preview of what’s coming: your scattered data goes in, a plan you can act on comes out.
Everything is gathered securely: studies, labs, wearables, doctor notes.
KOSMOS runs the deep analysis. About 12 hours, traceable back to sources.
You get an execution plan: insights, options, and questions ready for the next visit.
Information is everywhere, insight is nowhere. Studies, labs, wearables, and doctor notes are scattered across a dozen places, none of them talking to each other.
No one has time to synthesize it. Making sense of it all takes medical expertise and hours most people simply don't have.
Data without direction doesn't help. A pile of test results isn't a plan. It's just more to worry about.
Securely gathers data from every source: studies, labs, wearables, doctor notes.
AI agents synthesize millions of data points into what matters for you.
About 12 hours, up to 80% reproducible research.
Integrated with your full health profile: a clear, actionable plan, not a data dump.
Key insights. What is most important right now, and why.
Treatment options. Evidence-based options to discuss with your doctor.
Supplements & protocols. What may help, what to avoid, and why.
Tests to consider. Additional tests that could fill critical gaps.
Tracking & follow-up. What to monitor, when, and how to measure progress, shareable with doctors, caregivers, and family.
Your health data is encrypted in transit and at rest.
Built to meet HIPAA privacy and security standards.
Multi-agent deep research, ~12 hours per run, up to 80% reproducible, with every insight traceable to its sources.
Labs, portals, wearables, and notes flow into one research stream.
Plans export cleanly for doctors, caregivers, and family.

In a two-wave national survey, people with online access to their records reported more participation in their care, better clinician relationships, and more informed decision-making.
Reproducibility target of a KOSMOS deep-research run. Every insight in your plan traces back to the evidence it came from.
Brae programs build the physical, mental, and spiritual strength to keep showing up, whether you are giving care or receiving it.
A Brae week: small, repeatable practices with your team gently keeping score.
Pick a program built for the work you're carrying: body, mind, or spirit.
Build the habit with short daily practices and gentle accountability.
Your team sees the streak. Encouragement is part of the program.
Caregiving costs the caregiver. Strained caregivers carry measurably worse health outcomes than non-caregivers.
Self-care is the first thing cut. When someone else's needs fill the day, your own strength quietly drains.
Willpower alone doesn't hold. Habits last when a team sees them, not when one exhausted person tries alone.
Join a Brae health program matched to where you are, giving care or receiving it.
Short daily practices, streaks, and reminders that fit around caregiving, not on top of it.
Programs for caregivers and for the people they care for.
Track progress alongside your team. Encouragement is part of the design.
Higher four-year mortality among older caregivers experiencing strain versus non-caregivers: the documented cost of caring without support.
Regular physical activity reliably reduces depressive symptoms in adults: the core mechanism behind every Brae program.
HomeTeams keeps the people around you connected. Brae builds the physical, mental, and spiritual strength to keep showing up. There is a Brae program for the work you are carrying, and each one is customized specifically to you.
Build the physical and inner strength to meet life well.
In 100 days, build a healthier human, a better-trained dog, and a bond strong enough to keep both showing up.
Turn everyday spending into funding for the people you care for. Real money, earned passively at participating businesses, pooled toward whatever your team needs to cover.
A Team Fund filling itself from spending that was happening anyway.
Everyone links a card they already spend with. Nothing new to carry.
Cash back accumulates passively at participating merchants, tagged to the goal.
The fund covers the cost. Cash out via ACH or spend on a virtual card.
Care is expensive. Medical copays, prescriptions, the unexpected bill. The costs add up, and they rarely stop.
The burden falls on one person. Asking others to chip in is awkward and manual: Venmo chases, spreadsheets, one-off drives that burn everyone out.
The money is already being spent. A whole community's everyday purchases could fund the need. It's just never captured.
Connect the card you already spend with. No new card required.
Cash back at participating local and national merchants, with no coupons or QR gymnastics.
Route earnings into a shared Team Fund, earmarked for a specific need.
Cash out to a bank, spend on a virtual card, or let the fund cover the cost.
Funding, not fundraising. Recurring, passive earnings instead of one-off, exhausting drives.
Real money, not points. Cash out or spend anywhere the card works. Nothing to redeem.
Built for teams. Money is earmarked per team and per goal in a real ledger, so everyone knows what's for what.
Any care need. Medical, family, activities, emergencies: the goal is whatever the team decides. HSA accepted.
Trust by design. Double-entry ledger, reserve-backed balances, transparent per-team totals.
Card-linked offers, a double-entry ledger, and reserve-backed balances. Every visible dollar is redeemable.
Financial data is encrypted in transit and at rest.
Funds are earmarked per team and per goal, so everyone sees what's for what.
ACH cash-out to a U.S. bank, or a virtual card in Apple Pay / Google Pay.
Use HSA funds where eligible.

Average annual out-of-pocket spending by U.S. family caregivers. For many, roughly a quarter of their income. The cost of care is real money, every year.
What one HomeTeams family saves each month by sharing care that would otherwise be full-time memory care: $10,000 a month brought down to about $2,000 with a HomeTeam in place.
Every life has a story. Make sure it lives on. Your Legacy Agent gently guides you through meaningful conversations, one story at a time, and gathers them into a Vault your family can return to for generations.
One story session: a gentle prompt, a voice recording, a memory that now outlasts the moment.
The Legacy Agent asks one thoughtful question at a time, so no one faces a blank page.
The story is captured in their own voice, with photos, videos, and keepsakes attached.
It joins one timeline your family can return to for generations.
Stories get told once, then fade. Someday, someone you love will wish they could ask you one more question.
Memories are scattered. Photos, letters, videos, and keepsakes live in boxes, drives, and albums, never brought together.
Writing it all down feels impossible. Most people don't know where to start telling a whole life story.
Record your stories and memories, in your own words.
Add images and videos that bring your life to life.
Store letters, certificates, milestones, and key records.
Share what you believe, what you've learned, and what matters.
Build your family tree and preserve your heritage.
Save recipes, traditions, faith, and cherished moments.
Guided, not a blank page. Your Legacy Agent asks the questions so you never have to figure out where to start.
Everything, organized as one story. Voice, photos, documents, and keepsakes live together on a single timeline.
Memories, restored. AI enhancement brings faded photos back to life, always with you in control.
A gift that lasts forever. Children, grandchildren, and future generations can hear your voice and understand the journey that shaped your family.
Stories are encrypted and visible only to the people you choose.
Record naturally; transcripts are created automatically and stay searchable.
Faded photos enhanced and restored, always with you in control.
Voice, photos, documents, and keepsakes live together, alphabetical or chronological.
Durable storage designed for the next generation, not the next app cycle.
One HomeTeam, every device. Free to download.
Life-review and reminiscence therapy shows a large effect on late-life depression across 42 randomized trials of 3,361 older adults. Roughly one in two participants benefits.
Reminiscence practice significantly reduces depressive symptoms and raises life satisfaction in older adults, with the strongest gains after eight or more weeks of regular storytelling.
The fourth quarter of life is the most important part of any game, where life’s lessons, habits, and practices come together.
