### **Dr. Apurv Gupta has spent more than two decades in healthcare. Now he is helping HomeTeams understand and prove what happens when care leaves the building.**

[Healthcare](https://www.hometeams.io/playbook/one-person-managing-care) is remarkably sophisticated while a patient is inside it. There are physicians, nurses, specialists, care managers, medical records, medication systems, and processes designed to move information from one professional to another. Then the patient goes home. And much of that structure disappears.

Someone has to understand the discharge instructions. Someone has to remember the medication change. Someone has to schedule the follow-up appointment. Someone has to recognize when something does not seem right. Very often, that responsibility quietly transfers to a spouse, daughter, son, friend, or caregiver.

Dr. Apurv Gupta knows that transition well.

Before becoming a healthcare executive and transformation leader, Dr. Gupta spent years practicing as a hospitalist — caring for patients during some of the most complex moments of their lives. He later served in medical director and Chief Medical Officer roles before moving deeper into healthcare performance improvement, quality, clinical governance and care transformation.

He earned his medical degree from Brown University and a Master of Public Health from Harvard University.

That background gives him an unusually broad view of healthcare.

He has been the physician at the bedside.

He has been the leader responsible for how an organization performs.

And he has spent years asking how healthcare systems themselves can work better.

Now he is bringing those perspectives together at HomeTeams.

## **The Question Behind the Work**

As Vice President of Clinical Innovation & Research, Dr. Gupta leads clinical research, outcomes validation and clinical partnerships for HomeTeams.

His mandate centers on one deceptively simple question:

### **Does connected, team-based care actually make care better?**

HomeTeams was built around the belief that health is a team sport. But Dr. Gupta’s role is not simply to agree with that idea.

His job is to help prove it.

That means studying what changes when patients and families have better access to information, clearer responsibilities and stronger connections to the people helping carry care forward.

Do families better understand what happens next?

Does clearer coordination reduce confusion?

Can earlier support keep a small problem from becoming a larger one?

Can technology support clinicians without creating another burden?

Those are the kinds of measurable questions Dr. Gupta is helping HomeTeams answer.

## **What Happens Between Visits**

For Dr. Gupta, some of the most important parts of healthcare happen in the space traditional healthcare has struggled to see due to it’s lack of tools, staff, and resources.

The hours after discharge.

The days between appointments.

The conversation at the kitchen table after everyone gets home.

The daughter trying to remember what the physician said.

The spouse noticing that something has changed overnight.

The physical therapy exercises that still need to happen on Tuesday evening, long after the appointment ended.

> **“We have an opportunity to study something healthcare has historically struggled to see: everything that happens around the patient between traditional encounters.”**

Dr. Gupta has seen extraordinary care delivered inside hospitals. He has also seen how quickly responsibility shifts to patients and families when they leave. HomeTeams gives him an opportunity to connect those two worlds — and then measure what happens when people are better informed, better coordinated and better supported.

That is where **care comes home**.

Not because healthcare professionals stop mattering.

Because their work has to continue through the people living the plan every day.

## **Building the System Around the Person**

Healthcare has traditionally built systems around hospitals, clinics, insurers and providers.

HomeTeams is building the system around the person.

Health records, care information, schedules, resources and actions can exist alongside the people helping make those things happen. The individual remains at the center of the HomeTeam and controls who is involved and what each person can access. The patient, their family, their friends, their community are a critical part of the care team which is currently not being leveraged or coordinated. That leaves many aspects related to health and thriving untended to. HomeTeams has the power to change that, and we are studying how that impact is coming about so that others can learn about how to create the essential elements to support healing.

For Dr. Gupta, the opportunity is bigger than simply developing another healthcare technology.

It is an opportunity to study whether a different kind of infrastructure can change what happens around a person.

Because technology alone is not the goal.

Better care is.

## **Humanity and Performance**

That question is also deeply connected to another part of Dr. Gupta’s work.

He is the founder of [**A Loving Organization**,](https://alovingorganization.org/about) an initiative focused on how healthcare organizations can embed humanity, connection, learning and compassion into the systems through which people work and care for others.

![A Loving Organization Integrate Chart](https://prod.superblogcdn.com/site_cuid_cmtbk8rbh003k01xtwmwt4xhs/images/68c09f75d9fcceaf3822b062apurv-gupta-md-mph-1790280403643-compressed.png)

His work challenges an assumption healthcare has wrestled with for years:

That organizations must choose between extraordinary performance and extraordinary care for people.

Dr. Gupta’s work argues that those things can reinforce one another.

Compassion does not have to compete with accountability.

Connection does not have to compete with efficiency.

Technology does not have to compete with human relationships.

That philosophy is closely aligned with what HomeTeams is trying to build.

Technology can organize information.

It can clarify what happens next.

It can help people understand their role.

But ultimately, people care for people.

> They are able to do so better, easier, and more effectively when they have systems at their disposal to do it. This is the essential finding of our Loving Organization work -- those organizations who have figured out how to make their systems more loving (policies, workflows, technology, leadership, teams, etc.) are able to deliver incredible outcomes across the board -- quality, safety, patient experience, profitability, and staff experience.” Said Dr. Gupta.

## **Proving What Comes Next**

HomeTeams is built around a simple idea:

**A person should not have to navigate health alone. We know healing comes from connections, and that is what we want to study through HomeTeams.**

Dr. Gupta is helping turn that idea into evidence.

His work will help determine whether stronger coordination improves understanding, whether families feel better supported, whether clinicians experience fewer preventable gaps and whether connecting the people around someone can meaningfully change what happens next.

Healthcare already knows how to measure what happens inside its walls.

Dr. Gupta wants to help understand what happens beyond them.

### **Because the patient goes home. The care goes with them. And what happens next matters.**
